Deliver high-quality, patient-centered physical therapy care as a Home Health Physical Therapist. This contract position offers the opportunity to work directly with patients in their homes, supporting recovery and independence through comprehensive and individualized treatment plans. Schedule and Assignment Details:

• Full-time position, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (57 daily patient visits based on visit types)
• Travel within assigned territory; mileage reimbursement included

Key Responsibilities:

• Perform thorough physical therapy assessments, including OASIS as applicable
• Design and implement customized plans of care targeting mobility, strength, balance, and safety
• Deliver expert interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, pain management, and fall prevention
• Monitor progress, adjust treatment, and communicate updates to interdisciplinary team members
• Educate patients and caregivers on home exercise programs, safety, and adaptive equipment
• Ensure timely, accurate documentation in Home Care Home Base (HCHB)
• Engage in collaborative case conferences and ongoing professional development
• Adhere strictly to regulatory, agency, and Medicare standards

What We're Looking For:

• Active PT license in good standing in the practice state
• Minimum 1 year of recent home health physical therapy experience
• Prior use of HCHB preferred
• OASIS proficiency required
• Excellent clinical judgment, assessment, and communication skills
• Strong organization and time-management abilities
• Reliable transportation with valid drivers license and insurance
