Deliver impactful care as a dedicated Home Health Physical Therapist, providing essential rehabilitation services to patients in the comfort of their own homes. As a vital part of the care team, you will foster patient independence, support recovery, and help clients achieve their mobility and functional goals.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Active and valid Physical Therapist license (state of practice) in good standing
- Minimum of 1 year recent experience in home health physical therapy
- Familiarity with Home Care Home Base (HCHB) preferred
- OASIS assessment experience is required
- Excellent clinical judgment with a patient-centered approach
- Strong organizational, time management, and communication abilities
- Ability to work independently and reliably navigate a field-based schedule
- Reliable transportation, current drivers license, and auto insurance
Primary Responsibilities:
- Perform thorough physical therapy evaluations, including OASIS assessments for starts of care, resumptions, recertifications, and discharge as necessary
- Design and implement personalized treatment plans focusing on mobility, strength, balance, and safe ambulation
- Administer skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
- Frequently assess progress, adapting care plans to meet evolving patient needs, and communicate status changes to physicians and interdisciplinary staff
- Instruct patients and caregivers in safe practices, home exercise programs, and use of assistive devices
- Accurately complete all electronic documentation in Home Care Home Base (HCHB)
- Collaborate actively with nurses, therapists, social workers, and aides for holistic, coordinated patient support
