Deliver meaningful care every day as a dedicated physical therapist, working directly with patients in their homes. This contract opportunity is ideal for a motivated and compassionate clinician who thrives in an independent, field-based role and is committed to supporting functional independence and quality of life.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist license, in good standing
- At least 1 year of recent home health physical therapy experience
- OASIS experience is required
- Familiarity with Home Care Home Base (HCHB) preferred
- Demonstrates strong decision-making and clinical assessment skills
- Excellent communication, organizational, and time-management abilities
- Ability to work both independently and collaboratively
- Reliable transportation, valid drivers license, and auto insurance
Primary Responsibilities:
- Perform thorough physical therapy evaluations and OASIS assessments as needed
- Develop and implement individualized treatment plans that address mobility, strength, balance, and safety
- Deliver skilled interventions (therapeutic exercise, neuromuscular re-education, gait/transfer training, and more) to support patient goals
- Adjust therapy plans based on ongoing patient progress and team collaboration
- Educate patients and caregivers on exercises, safety, and home management strategies
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate with nursing, therapy, and other interdisciplinary team members
- Attend required case conferences and participate in ongoing education
- Comply with all relevant regulations and documentation standards
