Job Description
An exciting opportunity is available for an experienced and compassionate professional skilled in Home Health Physical Therapy (PT) to provide essential care for patients in their own homes. This contract position requires a dedicated and organized individual who excels at working independently, performing comprehensive physical therapy evaluations, and developing tailored treatment plans that enhance each patients mobility, strength, and overall wellness.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Active Physical Therapist (PT) license (in good standing)
- At least 1 year of recent, relevant home health physical therapy experience
- Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
- Required proficiency with OASIS assessments
- Excellent clinical assessment and decision-making abilities
- Strong communication, organization, and time management skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work autonomously in a field-based setting
Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS documentation as needed
- Design and implement individualized care plans to improve function, mobility, balance, and safety
- Administer skilled physical therapy interventions: therapeutic exercises, gait and transfer training, fall prevention, and neuromuscular re-education
- Educate patients and caregivers on home exercises, safe mobility, and use of assistive devices
- Update treatment plans and communicate changes in patient status to the care team
- Accurately document all visits and interventions within the HCHB platform
- Participate in case conferences, interdisciplinary meetings, and ongoing professional development
- Maintain compliance with agency protocols and federal home health regulations
