Job Description

Exciting opportunities await experienced Physical Therapists in home health care. Youll make a meaningful difference by helping patients regain independence and improve their quality of lifeall while enjoying the autonomy and flexibility of field-based work. In this role, you will assess patient needs, develop individualized treatment plans, and utilize your expertise to deliver skilled interventions within the comfort of your patients homes.

Schedule & Assignment:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Field-based with daily travel to patient residences (mileage reimbursement provided)
  • Caseload: Approximately 2532 points per week, typically 57 visits per day, based on visit types

Key Responsibilities:

  • Complete comprehensive physical therapy evaluations, including required OASIS assessments.
  • Develop, implement, and adjust individualized care plans focused on safe mobility, function, and patient-centered outcomes.
  • Deliver skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention.
  • Educate patients and caregivers regarding safe use of assistive devices, home programs, and ongoing exercises.
  • Maintain timely documentation using Home Care Home Base (HCHB).
  • Collaborate with an interdisciplinary home health team to ensure comprehensive, coordinated care.
  • Participate in care conferences, team meetings, and required continuing education.
  • Comply with all regulatory, agency, and Medicare home health standards.

Qualifications:

  • Active PT license in good standing in the relevant state
  • Minimum 1 year of recent home health field experience
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Required OASIS assessment experience
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work independently and demonstrate effective time management skills
