An opportunity is available for a dedicated and skilled Home Health Physical Therapist to provide patient-centered care in a field-based setting. This position involves working with patients in their homes, utilizing clinical expertise to support recovery, mobility, functional independence, and improve quality of life. The ideal candidate is compassionate, detail-oriented, and capable of working both autonomously and in collaboration with a diverse healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive PT evaluations and OASIS assessments as required.
• Develop and implement tailored therapy plans targeting mobility, balance, strength, safety, and pain management.
• Deliver skilled interventions including gait training, therapeutic exercises, neuromuscular re-education, fall prevention, and transfer training.
• Educate patients and caregivers on home exercise regimens, safety strategies, and the use of assistive devices.
• Monitor progress, adjust care plans appropriately, and keep care team and physicians informed of status changes.
• Document all assessments and interventions using Home Care Home Base (HCHB).
• Travel to patient residences within the assigned territory (mileage reimbursement provided).
• Collaborate with nurses, occupational and speech therapists, social workers, and aides.
• Participate in interdisciplinary meetings, case conferences, and ongoing trainings.
• Ensure compliance with regulatory and home health guidelines.

Qualifications:

• Active and unrestricted Physical Therapist (PT) license in state of practice.
• At least 1 year of recent home health physical therapy experience.
• OASIS experience required; proficiency in HCHB preferred.
• Strong assessment and clinical decision-making skills.
• Reliable transportation, current drivers license, and auto insurance.
• Excellent communication, organization, and independent work abilities.

$2,247 - $2,377/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
