An exciting opportunity is available for an experienced Home Health Physical Therapist to provide patient-centered care in the comfort of patients homes. This contract role plays a vital part in helping individuals improve their mobility, independence, and quality of life through skilled physical therapy interventions.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract duration
- Field-based, with daily travel to patient residences (mileage reimbursement provided)
- Caseload averages 2532 points per week (57 visits/day)
Key Responsibilities:
- Perform thorough physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, and Discharge as applicable)
- Design and execute individualized care plans targeting functional mobility, strength, balance, gait, pain management, and safety
- Deliver evidence-based interventions (therapeutic exercise, transfer training, gait training, neuromuscular re-education, fall prevention)
- Regularly assess patient progress, adjust treatment plans, and communicate with physicians and care teams
- Educate patients and caregivers on safe mobility, use of assistive devices, and effective home exercise routines
- Accurately complete documentation using Home Care Home Base (HCHB)
- Engage in collaboration with interdisciplinary clinical staff and participate in care conferences
- Comply with all regulatory, agency, and Medicare home health guidelines
Qualifications:
- Active Physical Therapist (PT) license in good standing in the state of practice
- At least 1 year of recent home health physical therapy experience (field experience strongly preferred)
- Familiarity with Home Care Home Base (HCHB) and OASIS required
- Strong clinical judgment, time management, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivated, independent, and adept at managing a flexible, field-based schedule
