Job Description
Experienced and compassionate Home Health Physical Therapists are needed to deliver exceptional care to patients in the comfort of their own homes. This opportunity involves helping clients improve mobility, strength, balance, and functional independence, while fostering positive patient outcomes through skilled, individualized therapy.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active Physical Therapist (PT) license (in good standing)
- At least 1 year of recent field-based experience in Home Health PT
- Expertise in OASIS assessments is required
- Familiarity with Home Care Home Base (HCHB) preferred
- Strong clinical judgement and functional assessment ability
- Valid drivers license, reliable transportation, and current auto insurance
- Self-motivated, organized, able to work independently
- Excellent communication and time management skills
Primary Responsibilities:
- Complete comprehensive patient evaluations and conduct OASIS assessments as assigned
- Develop, implement, and modify individualized therapy care plans focused on improving function, safety, and mobility
- Provide skilled interventions (therapeutic exercise, gait and transfer training, fall prevention)
- Educate patients and caregivers on exercises, use of assistive devices, and home safety
- Maintain accurate, timely documentation using HCHB software
- Collaborate with nurses and other therapy team members for coordinated care
- Attend case conferences, interdisciplinary meetings, and participate in ongoing education
- Adhere to regulatory standards and Medicare home health guidelines
