Job Description

Make a meaningful impact in patient care by providing expert physical therapy services within the home health setting. This contract opportunity involves working directly with patients in their homes, supporting their ability to regain and maintain optimal function and independence.

Schedule & Assignment Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Anticipated caseload: 2532 points per week (about 57 patient visits per day)
• Field-based visits to patient residences (mileage reimbursement provided)

Essential Qualifications:

• Current, active Physical Therapist (PT) license in good standing
• At least 1 year of recent experience as a Home Health Physical Therapist; recent field experience highly preferred
• Demonstrated proficiency using Home Care Home Base (HCHB) for documentation preferred
• Experience in administering OASIS assessments is required
• Strong clinical evaluation, treatment planning, and patient education skills
• Reliable personal transportation with a valid drivers license and auto insurance
• Excellent communication, organizational, and interpersonal abilities
• Independent work ethic and time management capabilities

Role Responsibilities:

• Conduct comprehensive physical therapy evaluations, including required OASIS assessments
• Develop and implement individualized care plans to enhance mobility, function, and safety
• Deliver skilled PT interventions: therapeutic exercises, neuromuscular re-education, gait training, and fall prevention strategies
• Adjust plans of care based on patient progress and communicate updates with interdisciplinary teams
• Educate patients and caregivers on exercise programs, safety, and use of assistive devices
• Document all care activities accurately in HCHB
• Participate in team meetings, case conferences, and required training per regulatory standards

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
