Job Description
Step into a meaningful physical therapy role providing individualized care to patients in their homes. This contract opportunity calls for an experienced Home Health Physical Therapist ready to deliver skilled interventions and champion patients' independence in a supportive, field-based environment.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessments for various stages of care.
- Design and implement personalized treatment plans focused on functional mobility, strength, balance, and pain management.
- Provide therapeutic interventions such as neuromuscular re-education, gait and transfer training, and fall prevention.
- Monitor patient progress, adapt plans as needed, and communicate effectively with physicians and the interdisciplinary care team.
- Educate patients and caregivers about home exercise safety, the use of assistive devices, and functional strategies for daily living.
- Document care promptly and accurately using Home Care Home Base (HCHB) electronic system.
- Collaborate in case conferences and interdisciplinary meetings to coordinate comprehensive patient support.
- Adhere to agency and Medicare home health policies and regulatory requirements.
Qualifications:
- Active and unencumbered Physical Therapist license in the state of practice.
- At least 1 year of recent Home Health PT experience (field experience preferred).
- Proficiency with Home Care Home Base (HCHB) strongly preferred.
- Demonstrated experience with OASIS.
- Strong assessment, clinical decision-making, and organizational skills.
- Reliable transportation, current drivers license, and auto insurance.
- Ability to manage an independent, flexible field schedule.
