Experienced Home Health Physical Therapist (PT) role available in Decatur, GA, offering an opportunity to provide skilled, patient-centered care in residential settings. This full-time, 13-week contract position requires delivering physical therapy services to improve patients mobility, strength, balance, and overall independence within their homes.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
• Develop and implement individualized treatment plans targeting functional mobility, gait, pain management, and safety.
• Provide skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
• Monitor patient progress and modify treatments as necessary, collaborating closely with physicians and interdisciplinary care teams.
• Educate patients and caregivers on exercises, safety protocols, assistive devices, and home exercise programs.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Participate in case conferences, interdisciplinary meetings, and ongoing professional development.
• Adhere to regulatory guidelines and agency policies, including Medicare home health standards.

Qualifications / Desired Experience:

• Valid Physical Therapist license in good standing for the state of practice.
• Minimum of 1 year recent home health physical therapy experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) preferred; OASIS experience required.
• Strong clinical decision-making and functional assessment abilities.
• Reliable transportation with a valid drivers license and auto insurance.
• Ability to work independently and manage a flexible schedule in the field.
• Excellent communication, time management, and organizational skills.

Schedule and Details:

• Monday Friday, 8:00 AM 5:00 PM.
• Caseload averages 2532 points per week (about 57 patient visits daily).
• Daily travel to patient homes in the assigned territory with mileage reimbursement.
• Electronic documentation system utilized.
