A rewarding opportunity is available for a skilled Home Health Physical Therapist (PT) to deliver patient-centered therapy services in a field-based setting. This position focuses on helping patients regain functional mobility, strength, and independence through tailored plans of care in the comfort of their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: Approximately 2532 points per week (typically 57 patient visits daily, based on visit type)

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments
• Develop and implement targeted treatment plans for mobility, safety, and pain management
• Deliver skilled interventions such as exercise programs, gait and balance training, and fall prevention
• Continually assess patient progress, modifying care as needed and coordinating with an interdisciplinary team
• Educate patients and caregivers on exercises, device usage, and injury prevention at home
• Accurately document all care and interventions in the Home Care Home Base (HCHB) system
• Participate in case conferences and adhere to regulatory home health standards

Desired Qualifications:

• Current Physical Therapist (PT) license in the applicable state, in good standing
• At least one year of recent home health physical therapy experience (field experience preferred)
• Proficiency in Home Care Home Base (HCHB) highly desired; OASIS assessment experience required
• Excellent clinical decision-making, functional assessment, and organizational abilities
• Reliable transportation, valid drivers license, and active auto insurance
• Ability to work independently and effectively communicate with patients, families, and healthcare team

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
