Job Description

An experienced Home Health Physical Therapist is needed to provide skilled physical therapy services directly in patients homes. This role focuses on improving patients mobility, strength, balance, and overall independence through comprehensive evaluations and personalized treatment plans. The position requires a self-motivated, organized professional comfortable working independently to deliver high-quality, patient-centered care.

Key Responsibilities:

  • Conduct thorough physical therapy evaluations, including OASIS assessments when applicable.
  • Develop and implement individualized plans of care targeting functional mobility, strength, balance, gait, pain management, and safety.
  • Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Monitor patient progress, adjust treatment plans accordingly, and communicate changes to physicians and care teams.
  • Educate patients and caregivers on exercises, safety, assistive devices, and home exercise programs.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary team members for coordinated care.
  • Comply with agency policies, regulatory requirements, and Medicare home health guidelines.

Qualifications:

  • Active Physical Therapist license in good standing for the state of practice.
  • At least one year of recent home health physical therapy experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical decision-making and functional assessment skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently while managing a flexible schedule.
  • Excellent communication, time management, and organizational abilities.

Schedule & Details:

  • Full-time, 13-week contract
  • Monday to Friday, 8:00 AM 5:00 PM
  • Caseload of approximately 2532 points weekly (57 patient visits/day)
  • Daily travel within assigned territory (mileage reimbursed)
