Job Description

An excellent opportunity is available for an experienced and compassionate Home Health Physical Therapist (PT) to make a meaningful impact by delivering high-quality care to patients in their own homes. This contract position is well suited for a motivated professional who thrives in a field-based setting and enjoys working both autonomously and as part of an interdisciplinary healthcare team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive patient evaluations, including OASIS assessments as needed.
  • Develop customized therapy plans of care targeting mobility, strength, functional independence, and safety.
  • Deliver skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention.
  • Monitor and document patient progress; modify care plans and communicate updates to physicians and team members.
  • Educate patients and caregivers on exercise regimens, safety, and use of assistive devices.
  • Maintain accurate records within the Home Care Home Base (HCHB) platform.
  • Collaborate closely with nurses, occupational and speech therapists, social workers, and aides.
  • Attend routine case conferences, interdisciplinary meetings, and continuing education as indicated.
  • Ensure compliance with all agency policies and Medicare home health guidelines.

Qualifications:

  • Current Physical Therapist (PT) license in good standing.
  • At least 1 year of recent experience in home health physical therapy; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) is highly desirable.
  • OASIS assessment experience is required.
  • Strong communication, clinical, and organizational skills.
  • Reliable transportation, valid drivers license, and automobile insurance.
  • Demonstrated ability to work independently in a flexible, field-based environment.
