Job Description

Deliver impactful care and support patient independence through expert physical therapy in a home setting. This contract opportunity is designed for an experienced, compassionate Home Health Physical Therapist who thrives in a field-based role, providing vital services that enhance mobility, strength, and quality of life for patients in their homes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time contract, 13 weeks
  • Daily travel within assigned territory (mileage reimbursement provided)

Required Qualifications:

  • Active Physical Therapist (PT) license in good standing for the state of practice
  • At least 1 year of recent Home Health PT experience (field work strongly preferred)
  • Experience with OASIS assessments
  • Proficiency in Home Care Home Base (HCHB) electronic documentation system preferred
  • Reliable vehicle, valid drivers license, and current auto insurance
  • Ability to work independently in a flexible schedule environment
  • Strong clinical assessment and communication skills

Core Responsibilities:

  • Conduct comprehensive physical therapy evaluations and OASIS assessments as required
  • Develop and administer personalized, functional therapy plans
  • Deliver interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
  • Educate patients and caregivers on home exercise regimes, safety, and equipment use
  • Accurately document care using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary care teams including RNs, OTs, SLPs, social workers, and aides
  • Attend case conferences, interdisciplinary meetings, and participate in professional development
  • Maintain adherence to agency procedures, regulations, and Medicare guidelines
