A rewarding opportunity is available for an experienced and compassionate physical therapist specializing in home health services. This vital role centers on delivering high-quality, skilled therapy directly to patients in their homes, facilitating improved mobility, functional strength, and independence while working collaboratively within an interdisciplinary healthcare team.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract commitment
Qualifications:
- Current, active physical therapist license in the state of practice (in good standing)
- At least one year of recent home health physical therapy experience preferred
- Strong proficiency with OASIS assessments; Home Care Home Base (HCHB) documentation experience preferred
- Demonstrated expertise in clinical decision-making, functional assessments, and individualized plan development
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent organizational, communication, and independent time-management skills
Key Responsibilities:
- Conduct comprehensive evaluations, including OASIS assessments as required
- Develop and implement individualized, evidence-based physical therapy plans focusing on safety, functional mobility, strength, and pain management
- Deliver interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
- Evaluate patient progress, adapt plans as indicated, and communicate with physicians and care team members
- Educate patients and caregivers on home exercises, safety, and use of assistive devices
- Collaborate and participate in interdisciplinary case conferences and meetings
- Maintain accurate, timely documentation using Home Care Home Base
- Ensure compliance with agency policies, regulatory standards, and Medicare requirements
