Deliver compassionate, expert care as a Home Health Physical Therapist, helping patients regain mobility and independence in the comfort of their own homes. This contract role is ideal for clinicians who are proactive and excel in a self-directed, field-based environment while maintaining a focus on outstanding patient-centered outcomes.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active Physical Therapist (PT) license in good standing
- At least one year of recent experience in home health physical therapy
- Proficiency with the Home Care Home Base (HCHB) documentation system preferred
- OASIS assessment experience required
- Reliable vehicle, valid drivers license, and auto insurance for daily travel
- Strong clinical judgment, organizational capability, and effective communication skills
Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS assessments as required
- Design and implement individualized care plans addressing functional mobility, gait, strength, pain management, and fall prevention
- Deliver skilled interventions, such as therapeutic exercises, neuromuscular re-education, and transfer training
- Monitor patient progress, modify plans as necessary, and maintain compliant electronic documentation
- Collaborate with a multidisciplinary home health team to ensure exceptional, coordinated care
- Educate patients and caregivers regarding therapeutic strategies, safety, and the use of assistive devices
- Attend case conferences and interdisciplinary meetings
