An opportunity is available for a skilled and compassionate Physical Therapist with strong home health experience. As a valued team member, youll deliver evidence-based physical therapy to patients in their homes, positively impacting quality of life and maximizing independence. This is a full-time, 13-week contract role designed for professionals who thrive in the autonomy of field-based work.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Primary Qualifications:

• Current and active Physical Therapist (PT) license in good standing
• Minimum of 1 year of recent home health physical therapy experience
• Familiarity with Home Care Home Base (HCHB) for electronic documentation (preferred)
• Required proficiency with OASIS assessments
• Adept clinical decision-making and functional assessment skills
• Reliable transportation, active drivers license, and valid auto insurance
• Ability to self-manage a field-based caseload efficiently
• Excellent communication and organization abilities

Key Responsibilities:

• Perform comprehensive patient evaluations, completing OASIS assessments where appropriate
• Develop and implement individualized plans of care focused on mobility and independence
• Administer therapeutic interventions: neuromuscular re-education, gait and transfer training, pain management, and fall prevention
• Modify plans of care as needed based on patient progress in coordination with interdisciplinary teams
• Educate patients and caregivers on safety, assistive device use, and home exercises
• Ensure accurate, timely electronic documentation
• Travel daily to patient residences; mileage reimbursement provided
• Attend case conferences and participate in required interdisciplinary activities

$2,129 - $2,252/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
