This rewarding contract opportunity calls for a skilled and compassionate Physical Therapist to provide comprehensive care, empowering patients to achieve their mobility and independence goals while working independently in a field-based setting.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload averages 2532 points per week (about 57 patient visits per day)

Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least one year of recent experience in home health settings
• Required: OASIS assessment experience
• Proficiency with Home Care Home Base (HCHB) electronic documentation preferred
• Strong clinical decision-making, organization, and communication skills
• Reliable vehicle, valid drivers license, and auto insurance

Key Responsibilities:

• Complete thorough patient evaluations, including all required OASIS assessments
• Develop, implement, and adjust individualized plans of care to address functional deficits in mobility, strength, balance, and pain
• Deliver skilled interventions: therapeutic exercise, gait and transfer training, fall prevention, and neuromuscular re-education
• Monitor patient progress, document all care in Home Care Home Base, and communicate changes to the care team
• Educate patients and caregivers on exercises, assistive devices, and safety practices
• Collaborate closely with nurses, therapists, social workers, and other care providers to coordinate care
• Participate in team meetings and maintain compliance with policy and regulatory guidelines
