Deliver transformative care as a Home Health Physical Therapist providing essential physical therapy services to patients in their residences. This contract role spans 13 weeks and is perfect for a compassionate, self-directed clinician who thrives in a field-based environment and values patient independence and high-quality outcomes.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist (PT) license in the state of practice, in good standing
- At least 1 year of recent experience in home health physical therapy (field experience preferred)
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Strong clinical reasoning, communication, and organizational skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work autonomously and manage a flexible field-based caseload
Role Responsibilities:
- Conduct comprehensive physical therapy evaluations, including required OASIS assessments
- Develop and implement individualized care plans addressing mobility, strength, balance, gait, pain management, and safety
- Deliver therapeutic interventions such as neuromuscular re-education, gait and transfer training, fall prevention, and pain management strategies
- Monitor and document patient progress, adjusting care plans as clinically indicated
- Educate patients and caregivers regarding therapeutic exercises, safety, assistive device use, and home exercise routines
- Accurately record care using Home Care Home Base (HCHB)
- Collaborate within an interdisciplinary home health team, participating in case conferences and continuing education
- Uphold all agency policies, regulatory, and Medicare compliance standards
