This full-time, 13-week contract role provides autonomy and the chance to work one-on-one with patients, utilizing your expertise to help them regain or maintain their highest level of independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete in-home physical therapy evaluations, including OASIS assessments as needed.
• Develop and implement customized treatment plans to address mobility, strength, balance, pain, and safety.
• Deliver interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
• Monitor patient progress and update care plans, communicating effectively with physicians and the care team.
• Educate patients and caregivers in safe exercises, use of assistive devices, and home exercise programs.
• Accurately document all visits and care provided in Home Care Home Base (HCHB).
• Collaborate with a multidisciplinary team to coordinate comprehensive patient care.
• Attend case conferences and participate in ongoing professional development.

Qualifications:

• Current Physical Therapist license in the state of practice, in good standing.
• At least 1 year of recent field-based Home Health PT experience required.
• Proficiency with Home Care Home Base (HCHB) preferred.
• Demonstrated experience with OASIS assessments.
• Solid functional assessment skills and clinical decision-making abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently with strong time management and communication skills.

$2,602 - $2,753/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
