This contract opportunity is ideal for experienced clinicians passionate about fostering patient independence and delivering high-impact care in a field-based setting.

Schedule/Hours:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Manage approximately 2532 points per week, typically 57 patient visits per day, depending on patient needs

Key Responsibilities:

• Complete in-depth physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as needed
• Develop and implement individualized plans of care emphasizing mobility, strength, safety, and quality of life goals
• Deliver skilled interventions: therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention
• Monitor clinical progress, update care plans, and communicate changes to the interdisciplinary team and physicians
• Educate patients and caregivers on safe mobility, exercise programs, and use of assistive devices
• Document all care promptly and accurately in the Home Care Home Base (HCHB) system
• Collaborate with nurses, therapists, social workers, and aides to coordinate comprehensive patient care
• Attend interdisciplinary meetings and participate in required continuing education
• Uphold regulatory standards, agency policies, and Medicare guidelines for home health

Qualifications:

• Active Physical Therapist (PT) license in good standing
• Minimum 1 year of recent home health PT experience
• OASIS and field-based PT experience required; proficiency in HCHB preferred
• Strong clinical assessment and communication skills
• Reliable transportation, valid drivers license, and current auto insurance

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
