Deliver essential care as a Home Health Physical Therapist Assistant (PTA), providing skilled physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist. This field-based opportunity is ideal for professionals who are organized, self-motivated, and thrive in an independent environment while maintaining high standards of patient-centered care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Full-time commitment
• Approximately 2532 points per week (typically 57 patient visits daily)
• Daily travel required within an assigned territory

Key Responsibilities:

• Deliver physical therapy interventions according to individualized care plans.
• Assist patients with therapeutic exercises, transfer techniques, mobility, gait, and balance training.
• Closely monitor and document patient progress, reporting status and changes to the supervising Physical Therapist and care team.
• Educate patients and caregivers on safety, assistive devices, and tailored home exercise programs.
• Maintain accurate, timely documentation using the Home Care Home Base (HCHB) electronic system.
• Actively participate in case conferences and collaborate with the interdisciplinary team.
• Ensure compliance with all agency policies and Medicare home health regulations.

Qualifications:

• Active PTA license in the state of practice (in good standing)
• At least 1 year of recent Home Health PTA experience (field experience is preferred)
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation processes preferred
• Strong clinical observation skills and excellent communication abilities
• Reliable transportation, a valid drivers license, and auto insurance
• Strong organizational and time management skills

$1,542 - $1,631/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
