Job Description

An experienced and compassionate Home Health Physical Therapist (PT) is needed to deliver skilled therapy services in patient homes, focusing on improving mobility, strength, balance, and functional independence. This role requires a self-motivated professional who excels in working independently while providing patient-centered care and collaborating with interdisciplinary teams.

Location: Colt, AR a community offering a peaceful environment with a close-knit atmosphere, ideal for professionals who appreciate a quieter setting with access to local amenities and natural surroundings.

Schedule/Hours:

  • Monday through Friday
  • 8:00 AM to 5:00 PM

Responsibilities:

  • Conduct thorough physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge.
  • Develop, implement, and modify individualized treatment plans targeting mobility, strength, gait, pain management, balance, and safety.
  • Provide therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention interventions.
  • Monitor patient progress, communicate with physicians and care teams regarding treatment adjustments.
  • Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs.
  • Maintain accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with nurses, occupational therapists, speech therapists, medical social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, interdisciplinary meetings, and continuing education.
  • Adhere to regulatory guidelines and agency policies.

Qualifications:

  • Active Physical Therapist (PT) license in the state (in good standing).
  • Minimum one year of recent home health physical therapy experience; field-based experience preferred.
  • Proficient with OASIS assessments; familiarity with Home Care Home Base (HCHB) preferred.
  • Strong clinical decision-making and functional assessment skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently with excellent communication, time management, and organizational skills.
