An opportunity is available for a skilled and compassionate Home Health Physical Therapist to deliver quality care directly in patients homes. This role is ideal for someone with a strong background in home health, who values autonomy and is committed to improving patients functional independence within the comfort of their own environment.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM (full-time, 13-week contract)
- Daily travel within your assigned territory (mileage reimbursement included)
Key Qualifications:
- Active Physical Therapist (PT) license in good standing for the state
- Minimum of 1 year of recent experience providing physical therapy in a home health setting
- Experience with OASIS assessment and documentation
- Familiarity with Home Care Home Base (HCHB) system preferred
- Excellent communication and organizational skills
- Reliable vehicle, valid drivers license, and current auto insurance
- Ability to work independently and seamlessly adapt to a flexible, field-based schedule
Primary Responsibilities:
- Complete comprehensive in-home physical therapy evaluations and OASIS assessments as needed
- Develop and implement individualized care plans focused on improving mobility, safety, and patient independence
- Deliver skilled interventions, including therapeutic exercise, neuromuscular re-education, gait training, and fall prevention
- Provide patient and caregiver education on exercise programs and safety strategies
- Maintain thorough and timely electronic documentation
- Collaborate closely with other healthcare professionals to ensure coordinated care
- Attend required team meetings and ongoing professional development sessions
- Comply with agency policies, regulatory requirements, and Medicare guidelines
