Job Description

A rewarding opportunity is available for an experienced Home Health Physical Therapist (PT) passionate about making a tangible difference in patients lives. The ideal candidate is organized, self-sufficient, and thrives in a field-based setting while collaborating with a multidisciplinary team.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Expectation of approximately 2532 points per week (averaging 57 patient visits daily)

Key Responsibilities:

  • Conduct thorough physical therapy assessments, including standardized OASIS evaluations
  • Develop and implement tailored care plans to address patient-specific mobility, balance, and functional goals
  • Deliver skilled physical therapy treatments such as therapeutic exercise, gait and transfer training, fall prevention, and pain management
  • Adjust treatment plans based on patient progress and communicate updates with physicians and team members
  • Educate patients and caregivers on home programs, safety, and use of assistive devices
  • Document care accurately and efficiently using Home Care Home Base (HCHB) system
  • Participate in interdisciplinary collaboration, case conferences, and continuing education activities
  • Observe all regulatory, clinical, and safety guidelines

Qualifications:

  • Active and unencumbered Physical Therapist license in the state of practice
  • Minimum 1 year of recent experience in Home Health Physical Therapy (field experience highly preferred)
  • Proficiency with Home Care Home Base (HCHB) and strong OASIS experience required
  • Strong clinical reasoning, organization, and autonomy
  • Valid drivers license, reliable transportation, and current auto insurance
  • Excellent communication and time management skills
