An exciting opportunity is available for an experienced and compassionate Home Health Physical Therapist to provide skilled therapy services to patients in their own homes. This contract role emphasizes quality, patient-centered care, requiring a clinician who excels in delivering individualized therapy plans to support patient mobility, safety, and independence.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least 1 year of recent Home Health Physical Therapy experience preferred
• Proficient with Home Care Home Base (HCHB) electronic documentation system
• Experience with OASIS assessments required
• Reliable transportation, valid driver's license, and auto insurance
• Effective communicator with strong functional assessment and clinical decision-making abilities
• Comfortable working independently in a field-based, flexible schedule

Key Responsibilities:

• Complete comprehensive physical therapy evaluations, including required OASIS assessments
• Develop and implement tailored therapy plans emphasizing mobility, strength, balance, and patient safety
• Deliver skilled interventions, including therapeutic exercise and gait, transfer, and fall-prevention training
• Monitor patient progress, adapting care plans as needed and reporting changes promptly to physicians and interdisciplinary team members
• Educate patients and caregivers on home safety, exercise strategies, and use of assistive devices
• Accurately document all care in HCHB, ensuring compliance with regulatory and agency requirements
• Collaborate with nurses, occupational and speech therapists, social workers, and aides as part of an integrated care approach
• Attend case conferences, interdisciplinary meetings, and required training sessions
