Job Description

Provide expert physical therapy services to patients in their homes as a Home Health Physical Therapist on a 13-week contract based in Cleveland, OH. This role involves delivering personalized, compassionate care that enhances patients functional abilities and quality of life in a home health setting.

Responsibilities:

  • Conduct thorough initial assessments to evaluate patients physical conditions, mobility, and functional limitations.
  • Develop and implement individualized treatment plans aligned with patient goals and physician directives.
  • Administer therapeutic exercises, manual therapy, and modalities to improve strength, range of motion, and independence.
  • Educate patients and families on proper exercise techniques, safe transfers, and use of assistive devices.
  • Monitor patient progress and adjust treatment plans as needed.
  • Maintain timely and accurate documentation in compliance with regulatory requirements.
  • Collaborate effectively with interdisciplinary healthcare teams to ensure coordinated patient care.
  • Adhere to professional and ethical standards while meeting state and federal guidelines.

Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited program.
  • Current valid state license or eligibility for licensure as a Physical Therapist.
  • Experience working in a home health physical therapy setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records software.
  • Strong clinical reasoning, critical thinking, and communication skills.
  • Compassionate, patient-focused approach with motivation skills to encourage patient engagement.
  • Knowledge of Medicare regulations and home health documentation standards.
  • Valid drivers license and reliable transportation to visit patient residences.
