Advance your career by providing high-impact, patient-centered care as a Home Health Physical Therapist delivering rehabilitation services directly in patients homes. This contract position offers the rewarding challenge of helping individuals regain mobility and confidence in the comfort of their own residences while exercising clinical autonomy and building strong therapeutic relationships.

Schedule:

• Full-time, 13-week contract
• Monday to Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (approximately 57 patient visits per day)
• Daily travel within assigned territory (mileage reimbursement provided)

Core Responsibilities:

• Complete comprehensive initial and ongoing physical therapy assessments, including OASIS documentation as appropriate.
• Develop and execute individualized care plans to improve mobility, strength, balance, and functional independence.
• Deliver skilled interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
• Evaluate progress, adjust plans of care, and communicate with physicians and the interdisciplinary team.
• Provide patient and caregiver education on exercises, adaptive equipment, safety, and home exercise programs.
• Accurately document all care in the electronic health record system (Home Care Home Base).
• Collaborate with nurses, occupational and speech therapists, social workers, and aides for coordinated, holistic care.
• Participate in team meetings and maintain regulatory and policy compliance.

Qualifications:

• Active Physical Therapist (PT) license in the state of practice, in good standing
• At least one year of recent experience in home health physical therapy
• Proficiency with OASIS assessments and preferably Home Care Home Base documentation
• Excellent assessment, problem-solving, and communication skills
• Self-motivated and highly organized, capable of managing a flexible, field-based schedule
• Reliable transportation, valid drivers license, and auto insurance

$2,839 - $3,003/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
