Job Description
Deliver skilled care in a rewarding field-based role as a Home Health Physical Therapist. In this contract position, you will serve patients directly in their homes, helping them achieve greater mobility, independence, and quality of life through evidence-based interventions and patient-centered care. Your expertise will be vital in empowering patients and caregivers, developing rehabilitation plans, and tracking clinical progress while collaborating as a valued member of an interdisciplinary team.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based with daily local travel, mileage reimbursement provided
Qualifications:
- Active Physical Therapist (PT) license in good standing
- Minimum 1 year of recent home health PT experience, field-based preferred
- Proficiency with OASIS assessments (Start of Care, Resumption, Recertification, Discharge)
- Comfortable with Home Care Home Base (HCHB) documentation system preferred
- Self-motivated, strong clinical judgment, and able to work independently
- Reliable transportation, valid drivers license and current auto insurance
Key Responsibilities:
- Conduct comprehensive PT evaluations and OASIS assessments in patients homes
- Develop and implement individualized care plans focused on mobility, safety, and functional goals
- Deliver skilled interventions such as gait and transfer training, exercise, fall prevention, and pain management
- Educate patients and caregivers on safe mobility, exercise, and the use of assistive devices
- Maintain accurate and timely documentation in HCHB
- Communicate effectively with physicians and interdisciplinary team members
- Attend team conferences and ongoing professional education
