Deliver patient-centered care and transform lives as a Home Health Physical Therapist providing essential therapy services in a field-based environment. Youll conduct comprehensive evaluations, develop targeted treatment plans, and support individuals in achieving greater functional independenceall from the comfort of their own homes. Your ability to work independently, maintain meticulous documentation, and collaborate with an interdisciplinary team will drive your success and positively impact patient outcomes.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Primary Responsibilities:

• Complete thorough physical therapy assessments, including OASIS documentation as required.
• Design and implement individualized care plans that address functional mobility, strength, balance, gait, pain control, and safety.
• Provide skilled interventions, such as therapeutic exercise, neuromuscular re-education, gait training, transfer assistance, and fall prevention strategies.
• Continuously assess progress, adapting interventions as needed, and keep physicians and team members updated on changes.
• Instruct patients and caregivers on exercises, home programs, safety protocols, and appropriate use of assistive equipment.
• Ensure accurate, timely clinical documentation within Home Care Home Base (HCHB).
• Actively participate in interdisciplinary meetings, case conferences, and ongoing education when required.
• Abide by all policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist (PT) license, in good standing, for the state of practice
• Minimum one year recent experience in Home Health Physical Therapy (field experience preferred)
• OASIS experience required; proficiency with Home Care Home Base (HCHB) preferred
• Reliable transportation, valid drivers license, and auto insurance
• Exceptional clinical, communication, and organizational skills
• Ability to manage an independent, flexible field schedule
