Job Description
Make a real difference as a Home Health Physical Therapist by providing caring, individualized rehabilitation services to patients in the comfort of their own homes. Play a key role in helping individuals regain mobility, reduce pain, and maximize their independence as they recover from illness or injury. This contract assignment offers the opportunity to work collaboratively with an interdisciplinary healthcare team within a supportive, patient-focused environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Current and active Physical Therapist (PT) license in good standing in the state of practice
- At least one year of recent home health physical therapy experience (field-based roles highly preferred)
- Proficient in Home Care Home Base (HCHB) EMR system preferred
- Required OASIS assessment experience
- Solid skills in clinical assessment, decision-making, and care planning
- Strong interpersonal, organizational, and communication abilities
- Reliable transportation, valid drivers license, and active auto insurance
- Self-motivation and ability to manage a flexible, field-based caseload
Key Responsibilities:
- Conduct detailed physical therapy evaluations, including OASIS assessments as required
- Develop and implement personalized therapy plans of care focused on restoring function and independence
- Deliver skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
- Monitor patient progress, updating care plans and collaborating with other clinicians as patient needs evolve
- Educate patients and caregivers on exercises, home safety, and adaptive equipment
- Document visits and interventions timely and accurately using HCHB
- Participate in interdisciplinary case conferences and staff meetings
- Adhere to organizational guidelines and Medicare home health regulations
- Travel within the assigned territory (mileage reimbursement provided)
