Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Duties:

• Complete comprehensive physical therapy evaluations, including OASIS assessments as needed.
• Develop and implement customized treatment plans focused on improving functional mobility, strength, balance, gait, pain management, and overall safety.
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
• Reassess patient progress regularly; adapt goals and plans in collaboration with the interdisciplinary team.
• Provide caregiver and patient education for home exercise routines, safe mobility, and the proper use of assistive devices.
• Maintain accurate records using the Home Care Home Base (HCHB) electronic documentation system.
• Participate in regular interdisciplinary meetings and case conferences.
• Uphold high standards of compliance within agency, Medicare, and state home health guidelines.

Qualifications:

• Active Physical Therapist (PT) license, in good standing, for the state of practice.
• At least one year of recent field-based Home Health Physical Therapy experience.
• OASIS assessment experience.
• Familiarity with Home Care Home Base (HCHB) preferred.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong clinical assessment, communication, and time management skills.
• Ability to work independently and manage a flexible schedule.
