Deliver exceptional patient care as a skilled Home Health Physical Therapist, providing individualized therapy services in patients homes and supporting their journey to improved mobility, strength, and independence. This contract opportunity requires a motivated and organized clinician who enjoys autonomy, values collaboration with an interdisciplinary care team, and strives for excellence in patient-centered outcomes.
Schedule/Hours:
- Monday through Friday, 8:00 AM 5:00 PM
Qualifications & Preferred Experience:
- Active, unrestricted Physical Therapist license in the state of practice
- At least one year of recent experience delivering Home Health Physical Therapy (field-based preferred)
- Proficiency with OASIS assessments for start of care, recertification, and discharge
- Familiarity with Home Care Home Base (HCHB) electronic documentation is strongly preferred
- Demonstrated skill in designing and implementing evidence-based treatment plans
- Capable of working independently in the field, with strong organizational and time management skills
- Reliable transportation, current drivers license, and auto insurance are required
Role Responsibilities:
- Conduct patient assessments, including OASIS, and establish individualized care plans
- Deliver hands-on therapy, focusing on mobility, strength, fall prevention, and pain management
- Educate patients and family members on safe home exercise programs and the use of assistive devices
- Monitor and clearly document patient progress, modify care plans as needed, and communicate ongoing needs to physicians and allied providers
- Collaborate efficiently with nurses, occupational therapists, speech therapists, and home health aides to ensure seamless care
- Maintain accurate, prompt documentation in HCHB and adhere to regulatory guidelines for compliance
