Deliver expert physical therapy services as a Home Health Physical Therapist (PT), empowering patients to regain mobility, strength, and independence within the comfort of their own homes. Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments.
- Develop and implement individualized treatment plans targeting functional mobility, gait, balance, and pain management.
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
- Monitor patient progress and adjust plans accordingly while maintaining clear communication with physicians and care teams.
- Educate patients and caregivers on home exercise programs, safety, and assistive device use.
- Complete accurate documentation using Home Care Home Base (HCHB) software.
- Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
- Comply with agency policies, regulatory standards, and Medicare home health guidelines.
Desired Experience and Qualifications:
- Current Physical Therapist license in good standing for the state of Arkansas.
- At least 1 year of recent home health physical therapy experience, preferably field-based.
- Proficiency with OASIS assessments is required; experience with HCHB preferred.
- Strong clinical assessment and decision-making skills.
- Reliable transportation with valid drivers license and insurance.
- Ability to work independently and manage a flexible schedule.
- Excellent organizational, communication, and time management abilities.
Schedule and Contract Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload of approximately 2532 points weekly (typically 57 patient visits per day).
- Daily travel within assigned territory with mileage reimbursement.
