Job Description
An opportunity is available for a skilled and compassionate Physical Therapist with home health experience to provide high-quality therapy services in patients residences. This field-based position focuses on maximizing clients independence, mobility, and safety, while collaborating with an interdisciplinary care team and supporting patient-centered goals.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active and valid Physical Therapist (PT) license in the state of practice
- At least one year of recent experience in Home Health Physical Therapy, with field experience strongly preferred
- Proficiency with the Home Care Home Base (HCHB) electronic documentation system is a plus
- Experience with OASIS assessments is required
- Strong clinical assessment, decision-making, and patient education skills
- Reliable transportation, current drivers license, and automobile insurance
- Ability to work both independently and collaboratively, managing a flexible schedule effectively
- Excellent organizational and communication strengths
Responsibilities:
- Conduct OASIS-based comprehensive patient evaluations and create individualized care plans addressing mobility, strength, balance, pain management, and safety
- Deliver skilled interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
- Regularly assess progress, adjust interventions as needed, and communicate updates with physicians and team members
- Educate patients and caregivers on home exercises, assistive devices, and safety strategies
- Complete timely, accurate documentation in HCHB
- Collaborate with nursing and therapy colleagues for thorough, coordinated care
- Attend meetings, case conferences, and required training sessions
- Remain compliant with Medicare, agency, and regulatory standards
