Job Description

A rewarding opportunity is available for a dedicated Home Health Physical Therapist (PT) seeking to make a meaningful difference in patients lives. This contract role involves delivering high-quality, patient-centered therapy services directly in patients homes, focusing on enhancing mobility, independence, and overall well-being.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations, including OASIS assessments for different phases of care (Start of Care, Resumption, Recertification, Discharge).
  • Develop and manage individualized care plans to address strength, mobility, balance, pain management, and safety.
  • Deliver targeted interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention.
  • Monitor patients progress, adjusting plans as necessary and collaborating with physicians and interdisciplinary team members.
  • Educate patients and caregivers on exercise routines, safety measures, and the use of assistive devices.
  • Complete timely documentation using Home Care Home Base (HCHB).
  • Attend case conferences, interdisciplinary team meetings, and participate in ongoing professional development.

Qualifications:

  • Active Physical Therapist (PT) license (state of practice, in good standing)
  • Minimum of 1 year of recent home health physical therapy experience; field experience preferred
  • OASIS experience is required; proficiency in HCHB preferred
  • Excellent clinical assessment, communication, and time-management skills
  • Reliable transportation with valid drivers license and auto insurance
  • Ability to work independently and efficiently manage a field-based workload
