Step into a rewarding opportunity as a Home Health Physical Therapist, providing individualized therapy for patients in the comfort of their own homes. This full-time, 13-week contract role focuses on improving patient mobility, strength, and independence with a strong emphasis on patient-centered care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Your Day-to-Day:

• Manage a caseload of approximately 2532 points weekly, averaging 57 visits per day
• Travel daily to patients within your assigned territory, with mileage reimbursement provided
• Conduct comprehensive assessments, including OASIS for Start of Care, Recert, Resumption, and Discharge as needed
• Develop and implement tailored care plans targeting mobility, pain management, safety, and functional goals
• Deliver skilled interventions such as neuromuscular re-education, gait and transfer training, therapeutic exercises, and fall prevention strategies
• Consistently communicate with physicians and interdisciplinary team members to monitor and adjust plans of care
• Educate patients and caregivers on home exercises, proper use of assistive devices, and safety procedures
• Maintain timely, accurate electronic documentation using Home Care Home Base (HCHB)
• Attend case conferences and interdisciplinary meetings to support collaborative care delivery
• Adhere to regulatory and agency standards, including Medicare guidelines

What Were Looking For:

• Active Physical Therapist (PT) license in good standing in Indiana
• At least 1 year of recent home health physical therapy experience
• OASIS assessment experience required; proficiency in HCHB preferred
• Strong autonomy, time management, and organizational ability
• Reliable transportation, valid drivers license, and auto insurance
• Outstanding communication skills and collaborative spirit

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
