Job Description
Seeking a skilled and compassionate Home Health Physical Therapist to provide high-quality, individualized therapy services to patients in their homes. In this contract role, you will play a vital part in helping patients regain mobility, balance, and independence, working closely with an interdisciplinary healthcare team.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments.
- Develop and implement personalized treatment plans addressing mobility, strength, balance, gait training, and pain management.
- Provide skilled interventions including therapeutic exercise, transfer training, neuromuscular re-education, and fall prevention.
- Monitor, assess, and adapt care plans based on patient progress, reporting changes to physicians and care team.
- Educate patients and caregivers on exercise programs, safety strategies, and assistive devices.
- Maintain timely, accurate documentation in Home Care Home Base (HCHB).
- Collaborate with nurses, occupational therapists, speech-language pathologists, and other care professionals.
- Participate in case conferences, interdisciplinary meetings, and ongoing education.
- Adhere to home health policies, Medicare guidelines, and regulatory standards.
Qualifications:
- Active and unrestricted Physical Therapist license in the state of practice.
- Minimum of 1 year of recent Home Health PT experience required; field-based experience preferred.
- Experience with OASIS required; HCHB proficiency preferred.
- Strong clinical, assessment, and independent decision-making abilities.
- Reliable transportation, valid drivers license, and insurance required.
- Excellent communication, time management, and organizational skills.
