Physical Therapists with home health experience are invited to provide patient-centered care and make a meaningful impactdirectly in patients homes. This contract opportunity involves working with a diverse caseload, providing clinically skilled physical therapy to adults seeking greater independence and mobility.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive physical therapy assessments, including OASIS documentation, in patient homes.
• Develop and implement individualized plans of care addressing functional mobility, strength, gait, pain management, safety, and balance.
• Deliver skilled therapy treatments: therapeutic exercise, neuromuscular re-education, gait training, and fall prevention strategies.
• Evaluate progress, adjust treatment plans as needed, and keep physicians and team members informed of patient status.
• Instruct patients and caregivers on safe exercises, functional activities, and use of assistive devices.
• Utilize Home Care Home Base (HCHB) for timely and accurate electronic documentation.
• Collaborate closely with nurses, rehab therapists, medical social workers, and home health aides for coordinated care.
• Participate in patient care conferences and regular interdisciplinary meetings.

Qualifications:

• Active Physical Therapist (PT) license, in good standing, for the state of practice.
• Minimum one year of recent home health physical therapy experience (field-based highly preferred).
• Proficient with OASIS assessments and experience with electronic documentationHome Care Home Base (HCHB) a plus.
• Strong clinical judgment, assessment, and communication skills.
• Reliable transportation, current drivers license, and valid auto insurance required.
• Able to work autonomously and manage a caseload across various patient home settings.

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
