This dynamic, full-time contract opportunity is ideal for a Home Health Physical Therapist (PT) who thrives working autonomously and values patient-centered care. You'll drive meaningful clinical outcomes while collaborating closely with an interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Current, active Physical Therapist (PT) license in good standing
- At least one year of recent field-based Home Health Physical Therapy experience
- Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
- Required proficiency with OASIS assessments
- Strong clinical assessment, decision-making, and organizational abilities
- Comfortable managing a flexible, field-based caseload
- Reliable transportation with valid drivers license and auto insurance
Role Responsibilities:
- Conduct thorough patient evaluations, including OASIS assessments
- Develop and execute personalized treatment plans focused on mobility, function, pain reduction, and safety
- Deliver skilled interventions (therapeutic exercise, gait, transfer and balance training)
- Routinely reassess patient progress and modify care plans as needed
- Educate patients and caregivers on exercises, safety practices, and adaptive equipment
- Collaborate with nurses, OT, SLP, social workers, and home health aides
- Document all patient care in Home Care Home Base
- Attend interdisciplinary meetings and case conferences as required
- Adhere to agency policies and Medicare home health regulations
- Travel daily within your assigned territory (mileage reimbursement provided)
