Job Description
Provide high-quality physical therapy in a home health setting, supporting each patients journey toward improved mobility, safety, and independence. As a Home Health Physical Therapist, youll deliver compassionate, individualized care within patients homes. Your expertise will drive functional outcomes through tailored interventions, education, and close collaboration with the broader healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Active Physical Therapist license in good standing (state of practice)
- At least 1 year of recent experience in Home Health Physical Therapy (field setting preferred)
- OASIS assessment experience required
- Familiarity with Home Care Home Base (HCHB) preferred
- Strong clinical judgment and functional assessment skills
- Reliable transportation, current drivers license, and auto insurance
- Ability to work independently and manage a flexible schedule
- Effective communication, time management, and organizational aptitude
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS/compliance assessments
- Develop and revise individualized care plans focused on improving mobility, strength, and safety
- Deliver skillful interventions: therapeutic exercise, gait/transfer training, neuromuscular re-education, and fall prevention
- Educate patients and caregivers on home exercise programs and safe use of assistive devices
- Maintain accurate, timely records through Home Care Home Base (HCHB)
- Collaborate with nurses, OT, SLP, social work, and home health aides to ensure well-coordinated care
- Attend case conferences, participate in interdisciplinary meetings, and uphold Medicare home health standards
