Deliver compassionate care where it matters most by providing physical therapy services in patients homes as a Home Health PT. This position offers the opportunity to work one-on-one with individuals, promoting recovery, independence, and enhanced quality of life. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including use of OASIS for appropriate assessments.
• Develop and implement individualized care plans emphasizing mobility, strengthening, safety, and reduction of fall risks.
• Provide skilled interventions: therapeutic exercise, gait training, neuromuscular re-education, transfer training, pain management, and fall prevention.
• Regularly assess patient progress, adapt plans as needed, and communicate changes to care team members.
• Educate patients and caregivers on exercise regimens, safe use of equipment, and techniques to increase independence.
• Complete timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with nurses, occupational and speech therapists, social workers, and aides for coordinated patient care.
• Participate in interdisciplinary meetings, case conferences, and required continuing education.

Qualifications:

• Active Physical Therapist (PT) license in the state of practice (in good standing)
• At least 1 year of recent field-based Home Health Physical Therapy experience
• Proficiency with OASIS assessments (required)
• Experience with Home Care Home Base (HCHB) preferred
• Excellent assessment, decision-making, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
