An outstanding opportunity is available for an experienced and compassionate physical therapist seeking to deliver patient-centered care in a home health setting. In this contract role, you will support individuals in regaining or maintaining mobility and independence in the comfort of their own homes. Your expertise and empathetic approach will help transform lives and guide patients to improved functionality and quality of life.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM to 5:00 PM
• Estimated caseload: 2532 points per week (approximately 57 patient visits per day, depending on visit type)
• Travel to patient homes required within your assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive physical therapy and OASIS assessments
• Develop, implement, and regularly update individualized therapy care plans
• Provide evidence-based interventions including therapeutic exercise, gait and balance training, transfer support, pain and fall prevention
• Educate patients/caregivers on effective exercises, safety practices, and use of assistive equipment
• Monitor progress, adapt plans as needed, and clearly document all care via Home Care Home Base (HCHB)
• Collaborate with an interdisciplinary healthcare team for optimal patient outcomes
• Participate in case conferences, continuing education, and uphold regulatory compliance

Qualifications:

• Current Physical Therapist license (in good standing) in the state of practice
• Minimum 1 year of recent field-based Home Health Physical Therapy experience
• Proficient with OASIS assessments; experience in Home Care Home Base (HCHB) preferred
• Excellent clinical reasoning, communication, and organizational abilities
• Self-motivated with reliable transportation, valid drivers license, and auto insurance

$2,602 - $2,753/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
