Job Description

Bring your expertise in home health and physical therapy to a dynamic role serving patients where theyre most comfortabletheir own homes. This position is ideal for a licensed Physical Therapist who is compassionate, self-driven, and adaptive to working independently while supporting positive outcomes for each patient.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Field-based with daily travel; approximately 2532 points per week (about 57 patient visits daily, varying by visit type)

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations, including OASIS assessments as needed.
  • Design and implement customized care plans targeting mobility, strength, balance, gait, and pain management.
  • Deliver evidence-based interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Track patient progress, modify plans as appropriate, and keep healthcare team and physicians updated on patient status.
  • Educate patients and caregivers on exercise routines, safety techniques, and the use of assistive devices.
  • Ensure accurate and timely documentation with Home Care Home Base (HCHB).
  • Work collaboratively with a multidisciplinary team to coordinate care and participate in case conferences.
  • Maintain compliance with agency procedures and regulatory standards, including Medicare requirements.

Qualifications:

  • Active Physical Therapist (PT) license in good standing
  • At least 1 year of recent home health physical therapy experience
  • OASIS assessment experience is required
  • Familiarity with Home Care Home Base (HCHB) preferred
  • Excellent assessment, communication, and organizational skills
  • Reliable transportation, current drivers license, and auto insurance
