Physical therapy professionals with a passion for delivering patient-centered care in home health settings will find this role both rewarding and impactful. In this full-time, 13-week engagement, you will provide essential physical therapy services to patients in their homesimproving mobility, strength, balance, and independence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Essential Qualifications:
- Active Physical Therapist (PT) license in good standing
- Minimum 1 year of recent home health physical therapy experience
- OASIS assessment experience required
- Proficient with Home Care Home Base (HCHB) documentation system (preferred)
- Strong functional assessment and clinical decision-making skills
- Ability to work autonomously while maintaining organized, high-quality care
- Reliable transportation, drivers license, and auto insurance
Key Responsibilities:
- Perform comprehensive patient evaluations, including OASIS assessments for start of care and other key milestones
- Create and implement customized care plans focusing on improving functional mobility and safety
- Administer therapeutic exercises, neuromuscular re-education, gait/transfer training, and fall prevention strategies
- Monitor and document patient progress, adapting plans as necessary
- Educate patients and caregivers on safe mobility, assistive devices, and home exercise routines
- Maintain timely, accurate records in HCHB
- Collaborate effectively with nurses, therapists, social workers, and aides for holistic care
- Attend interdisciplinary meetings and participate in ongoing professional development
- Ensure compliance with all policies and regulatory standards
