Job Description

As a Physical Therapist specializing in home health, you will provide skilled therapeutic services to adults throughout their recovery journeys. This contract role offers autonomy, professional growth, and the chance to deliver personalized, high-impact care.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

  • Active Physical Therapist (PT) license in good standing for the state of practice
  • At least 1 year of recent home health PT experience (field experience strongly preferred)
  • Proficiency with Home Care Home Base (HCHB) electronic documentation system preferred
  • Required experience with OASIS assessments for admission, follow-up, and discharge
  • Strong clinical assessment, decision-making, and organizational skills
  • Reliable personal transportation, valid drivers license, and auto insurance
  • Ability to excel independently within a field-based, flexible setting
  • Excellent communication, patient education, and time management abilities

Role Responsibilities:

  • Complete comprehensive PT evaluations (including OASIS)
  • Develop tailored treatment plans focused on mobility, strength, balance, pain management, and safety
  • Deliver evidence-based interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, fall prevention, and more
  • Monitor and modify care plans to achieve patient goals and communicate with care team members and physicians
  • Educate patients and caregivers on effective home exercise routines and the safe use of assistive devices
  • Maintain detailed documentation within HCHB and adhere to all regulatory and Medicare home health guidelines
  • Actively participate in interdisciplinary case conferences and continuing education
