Job Description
Embark on a rewarding role as a Home Health Physical Therapist, where your expertise will greatly impact patients mobility, independence, and quality of life. Your primary responsibilities will involve providing comprehensive physical therapy services within patients homes, developing and executing personalized treatment plans, and guiding patients and caregivers toward achieving optimal functional outcomes.
Schedule & Assignment:
- Full-time, 13-week assignment
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points per week (57 patient visits daily, based on visit types)
- Field-based position with daily travel throughout the assigned territory
Qualifications:
- Active Physical Therapist (PT) license, in good standing
- At least 1 year of recent experience in Home Health Physical Therapy (field-based experience preferred)
- Proficiency with Home Care Home Base (HCHB) documentation system is beneficial
- Required OASIS assessment experience
- Strong clinical judgment, functional assessment, and independent practice skills
- Reliable transportation, valid drivers license, and active auto insurance
- Effective communication, time management, and organizational skills
Key Responsibilities:
- Perform thorough patient evaluations, including OASIS assessments as needed
- Create and implement customized therapy plans to enhance mobility, strength, gait, and safety
- Deliver skilled interventions, such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies
- Continually assess progress, making modifications to plans of care as warranted, and collaborate with physicians and interdisciplinary team members
- Educate patients and caregivers on safety, proper exercise, use of assistive devices, and home exercise programs
- Accurately document all care in the Home Care Home Base (HCHB) system
