Deliver critical care to patients in their homes as a Home Health Physical Therapist, supporting individuals to achieve greater independence and improved function in a familiar environment. This contract position is ideal for a driven, organized clinician who enjoys autonomy while making an impactful difference in the lives of others.

Key Details:

• Contract: Full-time, 13-week duration
• Schedule: MondayFriday, 8:00 AM5:00 PM
• Caseload: Approximately 2532 points per week (typically 57 patient visits per day)
• Setting: In-home patient care, within assigned territory
• Documentation: Home Care Home Base (HCHB)
• Assessments: OASIS (for Start of Care, Resumption, Recertification, and Discharge)
• Daily travel with mileage reimbursement

Qualifications:

• Active, unencumbered Physical Therapist (PT) license in West Virginia
• Minimum of 1 year of recent Home Health Physical Therapy experience required
• Proficient with OASIS assessments and functional evaluation
• Experience with Home Care Home Base (HCHB) preferred
• Excellent clinical judgment, communication, and organizational skills
• Self-motivated and capable of managing a flexible, field-based schedule
• Reliable transportation, valid drivers license, and auto insurance required

Core Responsibilities:

• Conduct comprehensive physical therapy evaluations and OASIS assessments
• Develop, implement, and modify individualized therapy plans focused on mobility, strength, pain management, safety, and functional independence
• Provide skilled interventions including therapeutic exercises, gait training, neuromuscular re-education, and fall prevention strategies
• Educate patients and caregivers on exercises, equipment, and home safety
• Accurately document care using HCHB and participate in collaborative case conferences
• Maintain compliance with all regulatory and agency guidelines
