Schedule & Setting

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points weekly (57 daily visits based on acuity)
  • Field-based, traveling to patient homes within assigned territory (mileage reimbursement included)
  • Utilizes Home Care Home Base (HCHB) for documentation and OASIS for assessments

Key Responsibilities

  • Complete comprehensive assessments and develop individualized care plans centered on mobility, strength, balance, and safety
  • Deliver skilled interventions including exercise, neuromuscular re-education, gait/transfer training, and fall prevention
  • Evaluate progress and modify treatment plans; communicate with physicians and interdisciplinary teams
  • Educate patients/caregivers on home exercises, safety measures, and use of assistive devices
  • Accurately document all assessments, interventions, and patient responses using HCHB
  • Attend case conferences and participate in interdisciplinary collaboration and ongoing education
  • Adhere to agency policies and all applicable home health regulations

Qualifications

  • Active, unencumbered Physical Therapist (PT) license in the practicing state
  • At least 1 year of recent experience in home health physical therapy; field experience highly valued
  • Proficient with OASIS documentation; familiarity with HCHB preferred
  • Reliable transportation, valid drivers license, and auto insurance required
  • Excellent clinical skills, time management, and communication abilities

$2,247 - $2,377/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
