An opportunity is available for an experienced Home Health Physical Therapist to support patients in their homes, focusing on helping individuals regain mobility, strength, and safe functional independence. This role is ideal for a licensed PT who thrives in a field-based setting and excels at independent patient management and care planning.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: Approximately 2532 points per week (about 57 patient visits daily)
• Field-based, daily travel within assigned territory (mileage reimbursement provided)

Required Qualifications:

• Active Physical Therapist (PT) license in the state of Pennsylvania, in good standing
• At least 1 year of recent experience in home health physical therapy; field experience preferred
• Competency in using OASIS for Start of Care, Resumption, Recertification, and Discharge assessments
• Proficiency with the Home Care Home Base (HCHB) electronic documentation platform preferred
• Skilled in therapeutic exercise, neuromuscular re-education, gait training, transfer training, and fall prevention
• Strong functional assessment, clinical decision-making, and organizational abilities
• Excellent communication skills for collaborating with interdisciplinary teams and educating patients/families
• Access to reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

• Deliver comprehensive physical therapy evaluations and individualized care plans
• Implement patient-focused interventions to maximize mobility and functional goals
• Monitor and document patient progress, update treatment plans as needed
• Educate patients and caregivers on home exercise, safety, and assistive device use
• Maintain accurate, timely documentation in HCHB and ensure adherence to regulatory requirements
• Collaborate with nurses, OTs, SLPs, social workers, and aides for coordinated care

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
