Deliver essential physical therapy services to individuals in their homes while enjoying flexibility and autonomy in your workday. This contract opportunity is designed for an experienced physical therapist with a passion for home health and a commitment to high-quality, patient-centered care.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Expect daily travel to patient homes within the assigned territory

Key Responsibilities:

• Complete comprehensive physical therapy evaluations, including required OASIS assessments for skilled home health cases
• Design and implement individualized care plans to address mobility, strength, gait, safety, and pain management
• Deliver interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Assess progress, adjust treatment plans, and maintain clear communication with physicians and interdisciplinary teams
• Educate patients and caregivers on exercises, safety, and the use of assistive devices
• Ensure timely and accurate documentation using Home Care Home Base (HCHB)
• Collaborate closely with nurses, occupational and speech therapists, social workers, and aides to support holistic care
• Participate in meetings, case conferences, and ongoing education as needed
• Comply with regulatory guidelines and agency standards

Required Experience and Qualifications:

• Active Physical Therapist (PT) license (in good standing)
• Minimum of one year of recent home health physical therapy experience
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and insurance coverage
• Ability to work independently and manage a dynamic, field-based schedule
