Make a meaningful impact by delivering high-quality care directly in the comfort and familiarity of patients homes. As a skilled Home Health Physical Therapist (PT), you will facilitate improved mobility, strength, and independence for individuals needing home-based rehabilitation services.

Schedule & Contract Duration:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (typically 57 patient visits per day)
• Field-based position requiring daily travel within assigned territory

Key Responsibilities:

• Perform thorough physical therapy evaluations, including mandated OASIS assessments.
• Develop individualized care plans to address functional mobility, balance, gait, and pain management.
• Administer skilled interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Monitor and document patient progress, adapting treatment plans as needed.
• Educate patients and caregivers on safety, assistive device use, and home exercise regimens.
• Collaborate with interdisciplinary team members, participating in case conferences for coordinated care.
• Maintain accurate records using the Home Care Home Base (HCHB) platform.
• Uphold current regulatory and agency standards, including Medicare home health guidelines.

Qualifications:

• Active Physical Therapist (PT) license in good standing for the state of practice
• Minimum 1 year of recent field-based Home Health PT experience required
• Demonstrated proficiency with OASIS assessments and familiarity with HCHB preferred
• Excellent functional assessment, decision-making, and time management skills
• Reliable transportation, valid drivers license, and auto insurance
• Strong communication and organizational abilities
• Self-motivated, with the ability to work independently on a flexible schedule

$2,839 - $3,003/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
