Providing skilled physical therapy care in patients homes, this role focuses on improving mobility, strength, balance, and overall functional independence. The position is a full-time, 13-week contract based in Bridgeport, CT, offering a dynamic field-based work environment with a caseload of approximately 2532 points weekly.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments.
• Develop and execute individualized treatment plans targeting functional mobility, strength, balance, gait, pain management, and safety.
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Monitor patient progress, adjust treatment plans accordingly, and communicate updates with physicians and care teams.
• Educate patients and caregivers on exercise routines, safety techniques, assistive device usage, and home exercise programs.
• Maintain accurate documentation using the Home Care Home Base (HCHB) system.
• Collaborate with interdisciplinary healthcare professionals to ensure coordinated patient care.
• Participate in case conferences, team meetings, and continuing education.
• Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Current Physical Therapist (PT) license in good standing within the practicing state.
• Minimum one year of recent home health physical therapy experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) documentation preferred.
• Experience with OASIS assessments required.
• Strong clinical judgment and functional assessment capabilities.
• Reliable transportation with a valid drivers license and insurance.
• Self-motivated with excellent organizational, communication, and time management skills.
