Job Description

Deliver high-quality, patient-centered care as a dedicated Physical Therapist providing in-home rehabilitation services. This contract opportunity is ideal for an experienced clinician who thrives on fostering patient independence and enjoys the autonomy of field-based work. You will manage a diverse caseload and collaborate with an interdisciplinary team to deliver exceptional therapy outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive physical therapy assessments, including required OASIS evaluations for Start of Care, Resumption, Recertification, and Discharge.
  • Develop and execute individualized treatment plans emphasizing mobility, strength, functional safety, and pain management.
  • Deliver skilled interventions, including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
  • Track patient progress, adjust plans as needed, and maintain open communication with physicians and care team members.
  • Provide patient and caregiver education on assistive devices, safety precautions, and home exercise instruction.
  • Accurately document all visits and interventions using Home Care Home Base (HCHB).
  • Collaborate closely with nurses, occupational and speech therapists, social workers, and aides for coordinated care.
  • Participate in interdisciplinary meetings, continued education, and case conferences.
  • Adhere to all regulatory standards, Medicare guidelines, and agency protocols.

Required Qualifications:

  • Valid Physical Therapist (PT) license in good standing.
  • Minimum 1 year of recent Home Health PT experience.
  • OASIS experience (required).
  • Familiarity with HCHB (preferred).
  • Strong clinical assessment, communication, and organizational skills.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • Ability to work independently, efficiently manage time, and adapt to a flexible field-based schedule.
