Job Description

Deliver essential care to patients in their homes as a dedicated Physical Therapist. This full-time, 13-week contract position offers the opportunity to support individuals in regaining independence and functional mobility in a home health setting.

Schedule/Hours:

• Monday through Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• At least 1 year of recent experience in Home Health Physical Therapy (field experience strongly preferred)
• Solid understanding of Home Care Home Base (HCHB) software preferred
• Required experience with OASIS documentation and assessments
• Excellent clinical skills in patient assessment, care planning, and interventions
• Outstanding organizational, time management, and communication abilities
• Reliable vehicle, valid drivers license, and current auto insurance
• Ability to work independently while effectively managing a flexible patient caseload

Primary Responsibilities:

• Complete comprehensive physical therapy evaluations using OASIS as needed
• Develop, implement, and continually update individualized care plans for patients
• Provide therapeutic interventions including exercise, neuromuscular re-education, gait and transfer training, and fall prevention
• Regularly assess patient progress, modify treatment as needed, and communicate updates to the healthcare team
• Educate patients and caregivers on therapeutic techniques and safety strategies
• Maintain timely and accurate electronic documentation in HCHB
• Collaborate with interdisciplinary team members to ensure coordinated and effective care
• Participate in interdisciplinary meetings, case conferences, and ongoing education
• Adhere to all regulatory and agency guidelines
