An opportunity is available for an experienced and compassionate Physical Therapist with a background in home health care. This contract position focuses on providing essential skilled therapy services to patients in their homes, with the goal of restoring functional mobility, balance, and independence. The right candidate is organized, self-motivated, and comfortable working autonomously across a variety of patient needs while maintaining the highest standard of care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel within a designated territory (mileage reimbursement included)
• Expected caseload: 2532 points per week (approximately 57 visits per day)

Key Responsibilities:

• Conduct comprehensive patient evaluations, including OASIS assessments as applicable
• Develop and implement tailored plans of care based on each patients needs
• Provide skilled interventions such as therapeutic exercises, transfer training, and fall prevention strategies
• Monitor and document patient progress, adapting treatment plans as necessary
• Educate patients and caregivers on mobility, safety, and use of assistive devices
• Collaborate with nurses, therapists, and other healthcare team members to ensure excellent coordinated care
• Maintain up-to-date records utilizing Home Care Home Base (HCHB)
• Attend case conferences and interdisciplinary meetings as required

Qualifications:

• Active Physical Therapist license in the state of practice, in good standing
• Minimum of 1 year recent experience in Home Health Physical Therapy; field experience preferred
• Proficiency in Home Care Home Base (HCHB) preferred; OASIS experience required
• Strong clinical assessment, organizational, and communication skills
• Valid drivers license, reliable transportation, and auto insurance

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
