Deliver vital rehabilitation services and make a meaningful impact as a Home Health Physical Therapist. This 13-week contract role offers the opportunity to provide skilled, patient-centered care in a field-based setting, supporting individuals in achieving improved mobility, strength, and independence within their own homes. Working collaboratively with interdisciplinary healthcare colleagues, youll help drive outstanding patient outcomes while enjoying the flexibility and challenge of home health practice.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel to patient residences within your assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive PT evaluations, including OASIS assessments as needed
• Design and implement tailored therapy care plans focused on mobility, strength, pain management, and fall prevention
• Deliver skilled interventions: therapeutic exercises, gait and transfer training, neuromuscular re-education, and safety education
• Educate patients and caregivers on home exercise programs and safe use of assistive devices
• Document all care accurately and promptly using Home Care Home Base (HCHB)
• Communicate effectively with the care team and participate in interdisciplinary meetings
• Stay compliant with current clinical, regulatory, and agency guidelines

Qualifications:

• Active Physical Therapist (PT) license, in good standing
• At least 1 year of recent home health PT experience (field work preferred)
• Experience with OASIS assessments required
• Proficiency using Home Care Home Base (HCHB) preferred
• Strong clinical decision-making and organizational skills
• Excellent communicator with a patient-focused approach
• Reliable transportation, valid drivers license, and insurance required

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
