Job Description
A skilled and compassionate clinician is needed for a full-time, 13-week contract providing home health physical therapy services. This opportunity focuses on supporting patients mobility, functional independence, and overall quality of life. The clinician will deliver evidence-based care in a field-based setting, enabling patients to safely recover and thrive at home.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
In this role, you will carry a caseload of approximately 2532 points per week (typically 57 patient visits per day), with daily travel throughout your assigned territory. You will complete comprehensive assessments, utilize the OASIS tool as appropriate, and implement individualized plans of care using the Home Care Home Base (HCHB) electronic documentation system.
Key responsibilities include:
- Conducting patient assessments, including necessary OASIS evaluations
- Creating and modifying tailored therapy plans focused on mobility, gait, strength, and safety
- Delivering interventions such as therapeutic exercise, gait training, neuromuscular re-education, transfer techniques, and fall prevention
- Educating patients and caregivers on exercise programs, safety, and use of assistive equipment
- Coordinating with interdisciplinary team members to ensure holistic, coordinated care
- Maintaining timely, accurate documentation in HCHB and attending interdisciplinary meetings as needed
- Adhering to agency, state, and Medicare guidelines
Qualifications:
- Active PT license in good standing (state of practice)
- Minimum one year of recent home health PT experience
- OASIS assessment experience required; HCHB proficiency preferred
- Excellent independent clinical judgment and organization
- Reliable transportation, valid drivers license, and automobile insurance
